Evaluation Process
1. Fill out a caregiver
questionnaire which provides the therapists with information regarding
your child's past medical history, developmental history, feeding
history (if an area of concern) and a sensorimotor profile. In addition,
you would include your concerns and reasons for seeking out our
services.
2. After a review of
the questionnaire, the therapist will determine the specific methods
that would be appropriate for evaluating your child.
*Children ages 0 to 4 typically receive a
one and a half to two hour evaluation and those 5 and above require
approximately 3 hours to evaluate.
3. If you are wanting
us to bill their medical insurance a physician's prescription is
typically required for us to proceed with an evaluation and
treatment.

4. Once the intake is
received and reviewed by our therapists, an evaluation for your
child can be scheduled.
5. The child who is
0 to 3 years of age, will typically receive an evaluation in which
the therapist assesses the following areas:
Fine and gross motor skills
Sensorimotor functioning
Self-help skills
Oral motor/Feeding skills (if a concern)
6. The child/individual
who is 5 and older will typically receive an evaluation that includes
the assessment of the following:
Fine and gross motor skills
Visual motor skills
Visual perceptual skills
Ocular motor skills
Auditory perceptual skills
Sensorimotor abilities
Self-help skills
7. Once the therapist
has the opportunity to analyze the information received from the
standardized assessment, clinical observations and parent questionnaire,
they will complete a written report.
*Typically this process can take up to two
weeks.
8. Once the report is complete the evaluating therapist will meet
with the child's parents/caregivers to discuss the findings and
treatment recommendations.
